WHAT IS NEEDED: 1. Original birth certificate 2. Valid driver's license (Parents ID front and back) 3. Health Insurance card (A physical copy needs to be on file by the first practice) 4. $175 non-refundable registration fee* (*$25 TO HOLD SPOT│ Payment Arrangements are Available)
DEPOSITS: (REFUNDED WITH COMPLETION) $200 (check) for uniforms $200 (check) for volunteer time (6 hours/child) $200 (check) for fundraiser
(Deposits will be returned as long as everything is turned in and volunteer time is complete) DEPOSITS ARE TO BE CHECKS, THAT ARE NOT CASHED. ONCE EQUIPMENT IS TURNED IN AND VOLUNTEER TIME IS COMPLETE, YOU WILL GET THE CHECK BACK! PLEASE NO MONEY ORDERS FOR DEPOSITS!
